Chronic Disease Management

Making a positive impact on chronic diseases

Chronic disease is more common than most people expect. It includes medical conditions that have been present in patients and clients for more than six months, like diabetes, cardiovascular disease, arthritis, obesity, pulmonary conditions, multiple sclerosis or cancer.

It can also include chronic conditions like lower back or neck pain, or even depression and anxiety.

Exercise and diet applied clinically can make a real difference to managing and preventing these chronic conditions.

At ONTRAC we use both Exercise Physiology and Dietetics separately and in combination to treat, manage and prevent chronic disease and associated conditions. It’s called Chronic Disease Management – also known as CDM.

Treatments through Medicare, Private Insurers and DVA

We are authorised by Medicare, private health insurers and The Department of Veteran’s Affairs (DVA) to provide specialised treatments for a range of chronic diseases, including Type 2 Diabetes.

Why Exercise Physiology?

Exercise Physiology rests on the principle and practice of using exercise as medicine. Applied clinically, exercise has been proven to increase quality of life, speed up recovery and successfully prevent, treat and improve a range of health-related problems and conditions.

It can help with mobility, increase strength and movement, and assist with weight loss, blood pressure and other problems. Exercise Physiology contributes to higher overall levels of health and wellbeing.

What part is played by Dietetics?

Dietetics is all about good dietary habits and optimal nutrition. A good diet can also make a significant difference to a person’s health and wellbeing. Again it can help control blood pressure, contribute to maintaining a healthy weight and manage a range of symptoms often associated with chronic disease conditions.

Exercise Physiology and Dietetics in combination have been shown to have a positive impact on people with chronic diseases or conditions.

How can ONTRAC help me with my chronic disease management?

Our Accredited Exercise Physiologists (AEPs) provide specialist individual and group exercise physiology treatments to help you improve and manage a chronic disease.

Our Accredited Practicing Dietitians (APDs) develop meal plans, educate you on healthy eating habits and create and assist with selecting, buying and cooking the most nutritious food.

We create, manage and monitor clinical exercise and diet prescriptions in a safe and comfortable clinical environment. On their own, or in combination, they can make a very real difference to your quality of life and physical wellbeing.
